Learning Objectives

Upon successful completion of the program, participants will be able to:

  1. Describe emerging policy and care model trends shaping pharmacist-led chronic disease management in California.
  2. Identify payer and billing updates critical to the transition toward CRMC 2.0 models and new partnership opportunities.
  3. Analyze key performance metrics and engagement strategies from leading CRMC and heart failure care programs.
  4. Apply current evidence-based guidelines for diabetes (including GLP‑1 receptor agonists), hypertension, and dyslipidemia within comprehensive medication management (CMM) frameworks.
  5. Integrate effective patient recruitment and retention strategies shared by community health workers (CHWs) and clinical pharmacy technicians into existing chronic care workflows.
  6. Formulate actionable steps to enhance interprofessional collaboration, documentation quality, and patient outcomes through interactive session learnings and peer discussions
