Carla Blieden
Carla Blieden, PharmD, MPH, AAHIVP, has joined the faculty of the Titus Department of Clinical Pharmacy as assistant professor of clinical pharmacy, effective Oct. 1. She also serves as the director of student outreach and community health.
Blieden is a certified HIV pharmacist and has almost a decade of experience as a clinical pharmacist at the Maternal Child and Adolescent/Adult Center for Infectious Diseases and Virology, a family-centered HIV clinic at LAC+USC Medical Center. Her primary focuses at USC School of Pharmacy include integrating public health education into the curriculum and precepting students and residents at her clinical practice sites at LAC+USC Medical Center.
Connie Kang
Connie Kang, PharmD, BCPS, CGP, has joined the faculty of the Titus Department of Clinical Pharmacy as assistant professor of clinical pharmacy (clinician educator), effective Oct. 21. Kang comes to USC from the KGI School of Pharmacy and Health Sciences where she was an assistant professor since July 2014. Prior to that, she worked as a pharmacist in a broad range of practice settings.
Kang is a board-certified pharmacotherapy specialist and certified geriatric pharmacist and holds a Doctor of Pharmacy from University of the Pacific. She currently is finishing her Doctor of Education in Curriculum and Instruction at University of the Pacific. Her research interests include action research in the classroom and understanding student learning.
Tam Phan
Tam Phan, PharmD, has joined the faculty of the Titus Department of Clinical Pharmacy as assistant professor of clinical pharmacy (clinician educator), effective Sept. 16. He is also a community-based ambulatory care clinician at the Los Angeles LGBT Center and works with the Office of Diversity, Equity and Inclusion as a faculty advisor for Rx Pride, an LGBTQ+ affinity group.
As a PGY-1 Community-Based Pharmacy Resident, Phan served as adjunct faculty, giving didactic lectures, precepting and facilitating case conferences. While completing his PharmD, he led numerous patient care projects specifically focused on the underserved population and coordinated National Drug Take Back Day.
Raffi Svadjian
Raffi Svadjian, PharmD, MBA, has joined the faculty of the Titus Department of Clinical Pharmacy as assistant professor of clinical pharmacy, effective Oct. 1. Since 2017, he has served as the executive director of community pharmacies, supervising the operations of the five community pharmacies operated by the USC School of Pharmacy: USC Pharmacy, USC Health Center Pharmacy, USC Medical Plaza Pharmacy, Verdugo Hills Professional Pharmacy and Keck Medicine of USC Specialty Pharmacy.
Over his 16 years’ experience at USC Pharmacy, Svadjian has also held the positions of senior director of specialty pharmacy, director of transplant pharmacy services and staff pharmacist at USC Medical Plaza Pharmacy. His areas of interests include: prescription compounding, community pharmacy business management and the use of technology in community pharmacy.
