PGY1 Pharmacy at Ambulatory Care Clinics of Los Angeles Jewish Health/UCI SeniorHealth Center

This program will provide a well-rounded experience in healthcare settings serving the geriatric population. The primary site will be Los Angeles Jewish Health which services older adults in long term care and the assisted living space. Learning experiences will include participating in drug regimen reviews, developing chronic disease management protocols, and participating in a variety of interprofessional activities with healthcare professionals in those settings.

UCI Medical Center will serve as a participating site. At UCI, the resident will be embedded in a primary care geriatric practice with activities including geriatric assessments, chronic disease management, screening for high-risk medications and reducing polypharmacy as well as assisting providers and staff with typical primary care clinic issues such as authorizing refills, drug information questions, and drug coverage issues.
