CliniCare Pharmacy

CliniCare Pharmacy and Advanced Pharmacy Practice Center is one of the primary sites in the USC Mann PGY1 Community-Based Pharmacy Residency Program and it provides the resident with training and education at an independent pharmacy and care center located in the heart of San Fernando Valley. CliniCare Pharmacy provides compassionate and exceptional patient services to meet the needs of the community, one patient at a time. The resident will have the opportunity to provide comprehensive and integrative patient care through established SB 493 protocols and interdisciplinary, collaborative practices.

The home-base practice sites will include:

  • CliniCare Pharmacy
  • CliniCare Advanced Practice Care Center, providing Disease state management (DSM), Medication therapy management (MTM) and Transitions of care (TOC)
  • Internal Medicine Clinic

Clinical Services:

  • Compounding
  • Comprehensive medication management
  • Disease state management
  • Hormonal contraceptives
  • Internal medicine office – primary care
  • Medication reconciliation
  • Medication therapy management
  • Naloxone furnishing
  • Smoking cessation
  • Transitions of care
  • International travel health

Pharmacy Operations:

  • Participate in initiating, modifying, and discontinuing drug therapy, based on collaborative practice agreements or other treatment protocols consistent with the laws, regulations, and practice policies and procedures
  • Inventory Management
  • Perform Medicare, Medicaid and Medical Billing
  • Perform medication Prior Authorizations
  • Participate in initiating, modifying, and discontinuing drug therapy, based on collaborative practice agreements or other treatment protocols consistent with the laws, regulations, and practice policies and procedures
  • Participate in all Clinical, Operation and Quality Meetings

Administrative Responsibilities:

  • Establishing Immunization Clinics
  • Expanding Pharmacy Programs and Services
  • Independent Pharmacy Management
  • Policy and Procedure Updates
  • Quarterly Business Review Meetings

Scholarly Activities and Service:

  • Serve as preceptors for students on clinical rotations
  • Develop and facilitate case-based learning activities for pharmacy students
  • Deliver didactic lectures for pharmacy or pre-pharmacy students
  • Engage, coordinate, and precept in community activities such as health fairs and education outreach
  • Design and execute research projects pertaining to pharmacy practice

The resident’s schedule may vary depending on ongoing activities and may include various longitudinal and elective rotations throughout the year. Below is a sample of a weekly schedule:

  • Monday: MTM/Travel Clinic
  • Tuesday: Pharmacy Staffing
  • Wednesday: Internal Medicine Clinic/DSM/TOC
  • Thursday: Resident Development and Teaching Activities
  • Friday: MTM/DSM/TOC
